Direct Support Professional - Somerset and Hunterdon (NJ1360366)
This is a full time position paying between $11.50 - $11.75/hr up to 40 hours a week.
Support individuals as needed in daily living activities such as personal and health management, communication, self-advocacy, realizing personal and educational goals, meal planning and preparation, shopping, housekeeping, laundry, basic home maintenance and safety, recreation, home management, financial management and decision making. Support individuals as needed in developing friendships and establishing desired relationships with peers. Participate in the development of Individual Habilitation Plans and in support circle meetings as needed. Complete household chores to the extent necessary to maintain high standards of health, sanitation, safety and building maintenance at all times. Administer medication to individuals who need support taking medication. Attend monthly staff meetings and all required training programs as assigned by the supervisor. Complete all required paperwork accurately including, but not limited to, Critical Logs, IHP data, etc. Comply with Deltas policies and DDD licensing standards.
Resident Manager - Flemington (NJ1360373)
This is a full time position paying $14.71/hr up to 40 hours a week.
Promote and demonstrate ethical and professional conduct at all times ensuring employees conduct themselves in a similar manner. Participate in training, supervision, and evaluation of Community Support Associates and Advisors. Prepare and monitor staff schedule to ensure that appropriate coverage is available and that staffing is consistent with the budget for the support location. Complete individual assessments as required and assist in the development and implementation of Individual Habilitation Plans. Manage funds and maintain accurate records of expenses for individuals who entrust money to the company. Manage support location to maintain compliance with DDD licensure standards and company policies and procedures. Ensure staff maintain required trainings related to individual supports, staff development, and agency policies. Prepare all required reports such as Attendance, Monthly Report, Medication Count in a timely manner, as well as all other reports requested by Management Team or supervisor. Be accessible by phone, when necessary, to ensure the proper supervision of the program.
Behavior Support Specialist - Raritan (NJ1360399)
This is a full time position paying $14.00 - $14.72/hr up to 40 hours a week, Monday through Friday.
Participate in the development and implementation of high quality individual support plans. Promote and participate in Deltas Risk Management goals addressing the health and safety of customers and employees. Visit support locations to conduct observations, review records, collect data, and make recommendations as needed. Participate in planning meetings with support recipients; including IHPs and IDTs as needed. Provide on call crisis support to individuals and support staff. Implement action plans to address support needs identified in assigned referrals. Represent the company professionally in various venues, including, but not limited to, seminars, DDD sponsored forums, marketing activities and company events. Professionally communicate the companys vision, mission, core values and guiding support principles to support recipients, their families, employees, funding and accreditation personnel and the community. Perform other duties that may be assigned for providing excellent customer service and for the most effective operation of the company.
